W-9 Form

I noticed somewhere in reading that providers (freelance) must complete a W-9--is that correct? What about any other forms that have to be signed by the provider. Will oDesk make sure the provider submits these forms before a job, if needed?
I just want to make sure that any and all required forms that are to be completed by a provider of service are not omitted.
Advise please.

Thanks Michael. So I guess the best thing to do is submit a W-9 to oDesk. Is there a mailing address to submit the W-9?

Providers working in the US must submit a W-9 form before they can withdraw any earnings from oDesk. It doesn't stop work or billing, just withdrawal.
