Problem Ending assignment - no feedback links

I apologise if someone else has raised this and I have not spoted it.

I am happy with the job my provider has done and it is now time to close the assigment. I follow the menu for closing assignment untill I get to the page
with the drop down menu asking why assigment is ending.

Below that are some red asterixes with feedback catagories. But there does not seem to be any way of filling it in. There are no links or boxes to fill visible. I have tried three browsers. No wa to leave score either.

I have put the feedback in the Comments box, but that still has the form rejected as no feedback given.

Any ideas
