USA Providers Only: Submit a W9 Form

All providers based in the United States must submit an IRS form W9, using either your SSN, or if you are the manager of an incorporated affiliate firm, your EIN/Tax ID #. Without a W9 form, you will not be able to withdraw funds from your account. oDesk does not withhold taxes, but you may still be responsible for paying Local, State, and/or Federal Income Tax. Consult a local tax professional for more information.

Please download the oDesk W9 Form and return it via email (billing@odesk.com) or fax (650-853-4103).
