I need a simple web form created to take in potential partner information. Once submitted, I need the entered information to populate an access database (that also needs to be created). This is for 1 type of partner, eventually I'll need to accomodate 5 types of partners and need the access dbase built to accomodate this. There will be no more than 30 fields to capture.
