I need a site that will be for a contest. Any and all legal parts of the site will be provided by my attorney.

Basically when a contestant submits a form it will need to have their name along with their payment (via PayPal) sent to my legal team, with a randomly generated number.
The number will be the legal file number, but the number needs to be in a database (setup by you) so all we will reference on our end will be the number, but when a winner is picked we will need to be able to search the database with the number to reveal the name of the contestant.

The submit form will need to have:

name
address
city state zipcode country
age they must be over 18 years old
phone
a way that they can upload their "100 word essay" word, wordperfect, .pdf to us

there might be other things as well like actual site design, but this needs to be done and working in a week or less for the legal team's review
