Skills required: GoogleDocs Sheets formatting and formulas. If you have an eye for color, this would help too. The forms are excercize tracking sheets.

Work to be done: 
Phase 1: transfer scanned PDF pages into GoogleDocs, while replicating the formatting and implementing formulas by hand. The entire project is scanned in 18 pages, totaling 10 worksheets. The pages need to be entered manually, not converted by some software or automated procedure. 

This project is scheduled to start on Thursday, September 25th. If you intend to work on it I expect you to be available on that date.

Phase 2: make this a GoogleDocs template, so I and other people can fill it out. I want instruction on how to make this template made visible through GoogleDocs templates, so people can pick it up there.

Your bid will be rejected if your project cost exceeds $100, which is my top limit.
