I have a google doc form that collects name, address etc, and drops the data collected into a spreadsheet.
I want to improve on this form in both "layout" and "functionality"
Layout is too basic, such as:
- contains all the google "power by eyc" at the bottom
- First name on one line, then last name on next line, every box is on separate line. Where a well designed form would have some fields on the same line.
