I am building a website for the auto transport industry. The website is built for lead generation, so a large form is included at the top of every page.
I have no direction (colors, logo, etc), but rather an almost completely blank slate. Examples of companies in the same industry include:
I would like to build something professional and perhaps not just like everything company in the industry.

For SEO purposes pages cannot be just "splash" like those above, but have to have some text, but below the form. That said, the form is prominent and needs to be the first thing and the "call to action".
The site is ezusatransport.com. There is nothing there but the form and a main nav and footer nav. I am looking for someone to design the page layout from scratch and give input into things like navigational pieces and "module" layout, as well as base design (colors, logo, fonts, etc). Form layout and design is crucial (arguably number one goal) to get people to like the site and fill out the form is absolutely the most important thing. The end product would be a psd file that I will convert into HTML/CSS.
I want something really good, eye catching, action calling (fill out the form), comforting, etc. You must have a good portfolio so that I can see your work. You must be creative, I am not looking for a different colored version of the links I included above.
