Populate spreadsheet from scanned pdf forms

Job Description

I'd like someone to populate an excel spreadsheet template from hand filled in forms. An example of these forms are also attached (with sensitive information removed). There are roughly 120 forms extract the information from.

The columns required are -
Mail Offer (checkbox near bottom)
PDF File Name
Name (Parent/Guardian who signed the form)
Guardian 1 First Name
Guardian 1 Last Name
Guardian 1 Occupation
Guardian 2 First Name
Guardian 2 Last Name
Guardian Occupation
Mobile
Telephone
Email
Child's First Name
Child's Last Name
Birthday (day/mon/year)
