Three things need to be done in this task
1) To create auto-calculated field (using a very simple formula) in form or table
2) To create skip-pattern so if the total score is below 10 then automatically move onto N/A or stop the survey
3) I've created a form that include participants' names, follow-up dates, contact info etc. Then I created a query based on those info. What I want to do is once I found some info in query (for instance, If I see participant's name Jane and her follow-up date is due today) what do I need to find Jane's form in form without going through them one by one? Can I set it up in a way that I can find her form based on the information I have in query?
