GIS: map US congressional districts to US incorporated cities

Job Description

I'm looking to produce a report that would tell me what US cities map to which US Congressional districts.

Attached is a sample output for California—I'd like to have the same report for every state in the US.

I'm not fluent with GIS data from the US Census, but I've done some of the legwork.

Here's some of the data that I've found (because I don't know exactly what is needed to complete this, I've including everything that I've found. This is probably more than you need.)
