Make Excel Spreadsheet

Job Description

I need an Excel spreadsheet made to track the status and results of my leads. I need the spreadsheet to include the following fields:

-Date Lead received
-First Name
-Middle initial
-Last Name
-Address, City, Zip
-Phone Number
-Lender name
-Yellow letter sent? Yes or No selections available in a drop down box and date field
-Received call from letter? Yes or No selections available in a drop down box. A Date field and a Notes field
-Called back lead? Yes or no selections available in a drop down box. A Date field as well as another drop down box with available selections of Call 1, Call 2, Call 3, Call 4, Call 5. Notes field drop down box with available selections of Spoke to Lead, No answer, Left Message, Left Voicemail. Appointment set field drop down box with Yes or No selections available. A Date field. Notes field blank for typing in additional notes.
-Door knocked lead? Yes or No selections available in a drop down box. A date field as well as another drop down box with available selections of Knock 1, Knock 2, Knock 3, Knock 4, Knock 5. Notes field drop down box with available selections of Spoke to lead, No answer, DM not home, Left message, Left info/door hanger. Appointment set field drop down box with Yes or No selections available. A Date field. Notes field for typing in additional notes.
-Next steps field for typing in additional notes
-Status field drop down box with available selections of Active, Pending, Dead

The spreadsheet needs to allow for fields to be manually typed into as well as the ability to have information imported in to the fields.

I need the spreadsheet done in the next couple hours as I have to present it in a meeting tomorrow morning 2/14/13.
