mail merge from excel doc into word format

I get the same data every month in excel

I want a script to put the fields into a letter the same way every time.

Business Name

So that the letter is addressed and signed off the same way automatically. I want the address to show through the window of a DL envelope like this one

sample excel file is attached.

Should be a simple job for someone who knows mail merge excel and word.
